How do I apply chi square to my I.A?

Hello,

I have been trying to figure out how Chi squared works for a while. I looked at examples and videos but I don't know how it applies to my data. I am especially confused on how to make a contingency table?

 

I understand that my null hypothesis will be: McDonalds locations per 100,000 people and obesity rates are independent and then

My H1 is that: McDonalds locations per 100,000 people and obesity rates are dependent. 

I don't understand how one gets the values of the contingency table. 

I don't want it done for me but it would really help to get a step by step guidance.
